TRANSPORTATION FOR ERRANDS
DHS Health Service is pleased to introduce Transportation for Errands as a new Service we offer to clients. Home and Personal Support Workers insured by DHS can drive clients and accompany clients to medical and social appointments. Clients must not have any cognitive or physical impairment that would interfere with their ability to safely transport in a workers car, and they must be able to transfer independently to a workers car, which is not wheelchair accessible. The client must complete a Drivers Agreement Form authorizing the DHS worker to transport them in the workers car. Clients are responsible to pay for the workers services plus mileage and any parking incurred during the transportation. Transportation can include:
